Types of Care at a Life Plan Community
The Life Plan model offers a continuum of care for residents. Many Life Plan Communities include:
- Independent living. Independent living involves the least amount of care, while living in a community of other seniors. You'll have access to all amenities, activities, and dining options while living independently in your own apartment.
- Assisted living. Assisted Living provides support for daily living such as dressing and personal hygiene, meals, and medication management.
- Memory support. Memory support is specialized support for seniors who progress to more care due to cognitive decline or dementia.
- Skilled nursing care. Skilled nursing care is round-the-clock nursing care. Not all Life Plan Communities provide skilled nursing care for residents.
What is the Cost Structure at a Life Plan Community?
Because a Life Plan Community is your forever home there are financial considerations.
Most Life Plan Communities require an upfront entrance fee. The entrance fee secures your care for the long term. It ensures that if your needs change you will have access to care and services without significantly increased costs. In addition, they charge a monthly fee.
Horizon House’s entrance fee is based on what you can afford and which apartment model you select. In 2025, Horizon House’s lowest entrance fee is just over $64,000 with monthly fees starting at just over $2,000
The Horizon House cost structure stands out in another way as well. If any resident outlives their funds, and cannot pay the monthly fee, the Horizon House Residents Assistance Program is available. This program provides confidential financial support to current residents, as part of the Horizon House Partners in Caring Endowment.
