The Care Navigator offers:

Guidance to residents and their loved ones to ensure important documents are in place including durable powers of attorney, and advanced directives.

Assistance in managing new medications by enrolling in a medication management program.

Help in arranging additional services such as support with personal care, extra housekeeping, or special meal considerations.

Providing resources and support for caregivers whose partners have dementia.
