Our highly skilled Care Connector is a professional dedicated to the support of independent residents who may be experiencing decline in physical and/or cognitive abilities. The Care Connector also addresses partners who are serving as primary caregivers and provides programs—such as the Memory Café—that support inclusion for those experiencing memory loss.
A nurse is available 24/7 for medical emergencies and is dispatched when a resident contacts our reception desk or activates their pendant or pull cord.
Available to eligible residents for in-home care for an illness or injury. Examples may include monitoring serious illness or unstable health status, wound care, new user of supplemental oxygen, or newly diagnosed diabetes.
Home health nursing and/or therapies require a primary care provider order which is covered by insurance.
To enable independence and safety, home care services offer support with everyday activities like bathing, dressing, meal preparation, and household tasks.
Home Care agencies also provide nurse delegators. They are registered nurses who visit the home and train home-care aides to perform tasks such as administration of prescription medications or blood glucose testing. The RN visits at established intervals, based on need, to assess the resident’s condition.
Home Care does not require a provider’s order and can be self-initiated. Private pay service and some Long-Term Care insurance plans cover a portion of the home care costs.
